Please ensure you have been through the Research Ethics website, especially the FAQs section, so you have an understanding of what is required from you before completing an application form.

Low Risk Applications

Low Risk applications made via this process are those applications that are considered Low Risk and do not fall within a designated chapter (see FAQs for more detail).

Low Risk applications are considered on an ongoing basis ie. there is no set submission date as they are not reviewed by the full committee.

Applications are made via a QUT Low Risk Human Ethics Application Form:

  1. Download the Low Risk Application Kit (compressed ZIP folder)

  2. Save to your PC in a suitable location

  3. Extract files and open the 'Read me First' file

Committee Applications

Committee applications are those applications that are more than Low Risk and/or fall within a designated chapter; and/or are Low Risk but fall within a designated chapter. Please see FAQs for more detail on these designations.

Committee applications are considered at a sitting of the University Human Research Ethics Committee (UHREC). The following drop down list provides the dates submissions are required to be received by the Research Ethics Unit

Applications are made via the National Ethics Application Form (NEAF).

Note - If you have assessed your application as being Low Risk but falling within a designated Chapter you may complete an application using the Low Risk Application Kit. The application will, however, still need to be reviewed by the committee.

  1. Download the Committee Application Kit (compressed ZIP folder)

  2. Save to your PC in a suitable location

  3. Extract files and open the 'Read me First' file

Variations

If it is necessary to change your research protocol you must submit details of the changes including justification for the change and any relevant documentation, so that the variation can be reviewed and approved prior to use. Please submit a tracked changes version of documentation to highlight changes being made - this may include the application form, participant information documents, data collection instruments etc. Where multiple changes are being made a brief summary of the changes should be provided for ease of review.

Submission should be made via email to ethicscontact@qut.edu.au (if there are multiple files please ZIP them for emailing).

Annual & Completion Reports

 Human progress report: A report is required annually from the date of project approval, and upon completion of the project . Researchers will be sent a reminder containing a link to this page.
